NADRC Webinar on Care Navigators for People Living With Dementia and Their Caregivers

Join the ACL-funded National Alzheimer’s and Dementia Resource Center (NADRC) for a webinar on the use of care navigators for people living with dementia and their caregivers.

Care navigators are valuable members of interdisciplinary dementia care and support teams as they help people living with dementia and their caregivers connect with needed resources and provide important insight and information about their clients to the full dementia team. In this webinar, participants will learn about two programs using care navigators.

CAlz Connect, an ACL-funded initiative through the California Department of Aging (CDA), offers dementia care management services to people living with dementia and their caregivers in Imperial, Marin, and Ventura counties, using the Care Ecosystem and HomeMeds evidence-based programs. CDA’s presentation will provide participants with information on how they are collaborating with various partner organizations to launch CAlz Connect and the important role being played by community health workers who serve as care navigators.

In Kansas, OCCK, Inc. is implementing Care Ecosystem in various rural communities with its ACL funding. Within the care ecosystem, cognitive care navigators play a crucial role, helping the team understand the day-to-day issues experienced by people living with dementia and their caregivers and providing needed services and supports. They also assist in creating supportive partnerships within the communities they serve. OCCK Inc.’s presentation will explore the impact of the cognitive care navigator on their clients and their work with the full dementia care team. 

NADRC webinars are free of charge and open to the public, although pre-registration is required. The webinars are recorded and will be available for future viewing on the NADRC website. Closed captioning is available during the webinar and will be included in the recording.
