Connecting People to Services

ACL believes that home- and community-based services and supports should be robust enough that individuals with even the most complex medical and behavioral needs can be appropriately and effectively supported in their own home or other community-based settings. We know this is possible – it has been demonstrated in many communities.

Several ACL programs help older adults and people with disabilities find supports and services that they need in order to successfully live in their communities.

  • Aging and Disability Resource Centers Program/No Wrong Door System is a collaborative effort with ACL, the Centers for Medicare & Medicaid Services, and the Veterans Health Administration to support state efforts in streamlining access to long-term services and support options for older adults and people with disabilities
  • Centers for Independent Living (CILs) – CILs are community-based non-profit organizations that are consumer controlled. Although specific programs and services vary from center to center, all CILs provide information about, and referral to, services and supports available in the local community. 
  • Eldercare Locator is a nationwide service that connects older adults and their caregivers with trustworthy local support resources, including meals, home care or transportation, or a caregiver needing training, education or a well-deserved break from caregiving responsibilities.
  • Medicare Improvements for Patients and Providers Act provides grants to states and tribes to help older adults, people with disabilities, and caregivers apply for special assistance through Medicare.
  • State Health Insurance Assistance Program provides Medicare beneficiaries with information, counseling, and enrollment assistance.
