New Business Acumen Case Studies

October 16, 2017

The Center for Health Care Strategies (CHCS) has developed several case studies about creating partnerships between community-based organizations (CBOs) and health care organizations. One case study features five area agencies on aging: Eastern Virginia Care Transitions Partnership.

In a blog post, CHCS shares themes contributing to partnerships' success based on examining 200 collaborations:

  • Aligned missions and common values;
  • Complementary areas of expertise and balanced collaboration;
  • A well-structured referral process; and
  • Open communication among partners.

They also looked at challenges, such as building capacity, estimating resource needs and costs, developing and tracking performance metrics, and securing sustainable funding.

Read the case studies for more details about partnerships to support transitional respite care, reducing hospital readmissions for other adults, and addressing other needs through CBO and health care organization partnerships.
